IMMUNIZATIONS

12th GRADE- THE IMMUNIZATION REQUIRED BEFORE THE FIRST DAY OF SCHOOL IS THE *MENINGITIS (MCV4). THIS IS NOT THE SAME THING AS THE MENINGITIS B WHICH IS JUST RECOMMENDED. ALSO YOUR CHILD MUST HAVE BOTH OF THE REQUIRED *HEPATITIS A IMMUNIZATIONS.

PROOF OF HAVING RECEIVED THESE IMMUNIZATIONS MUST BE RECEIVED BY THE SCHOOL NURSE BEFORE THE FIRST DAY OF SCHOOL.

PARKING PASSES WILL NOT BE APPROVED FOR STUDENTS WHO HAVE NOT PROVIDED PROOF OF UP-TO-DATE IMMUNIZATION.

11th GRADE- IT IS HIGHLY RECOMMENDED THAT STUDENTS IN 11TH GRADE GET THEIR 12 TH GRADE IMMUNIZATIONS TO PREPARE AHEAD OF TIME FOR THEIR SENIOR YEAR.

MEDICATION

All medications given at school must have a consent form signed by a parent/guardian.

Parents must provide the medication for their child and the medication must be in the original container, medication in baggies or unmarked containers will not be accepted.

Students who carry inhalers need to have an Asthma Management Plan completed and signed by both parent/guardian and health care provider. Please access the Asthma Management Plan form by clicking on the Forms menu.

Please remember to pick up any unused medication from the school at the end of the school year. Medication cannot carry over to the next school year.

EXPERIENCE

I have been a registered nurse for 25 years. I am a Purdue graduate. I started my career in the Intensive Care and Open Heart units at Porter Memorial Hospital. I transitioned to Emergency Medicine and became an E.M.T. to further my education. I had a number of certifications to care for adult and pediatric trauma patients including Advanced Cardiac Life Support. I also worked for 8 years as a certified Oncology/Chemotherapy nurse and felt truly blessed to work with such a special group of patients. I was a substitute school nurse prior to working here and enjoyed the environment and especially the students so decided to make school nursing my career.
